• I acknowledge that my child's participation in this event and activities is not required by The Father’s House of Rochester New York (“TFH”), or any employee or volunteer; and that my child is considered medically fit to participate by their physician. I understand that TFH has done their best to investigate the safety of the equipment to be used or involved with for the event.

• I understand that their participation in sports and activities may involve an element of risk and danger of accidents. Knowing those risks, I hereby waive, release, and discharge The Father’s House of Rochester New York (“TFH”), its officers, directors, employees and/or volunteers from any and all liability, claims for damages for personal injury, death, or property damage, or any rights of recovery which I may have, or which may hereafter occur, as a result of their participation in this event sponsored by TFH. Knowing those risks, I hereby agree to voluntarily assume responsibility for those risks, known or unknown, and whether caused by negligence or accident. I further agree to indemnify and hold harmless TFH from any loss, liability, damage or costs, including court costs and attorney’s fees, that may incur as a result of their participation. Finally, I further agree that this waiver, release and assumption of risk is to be binding on my heirs and assigns.

• I understand that, in the event medical treatment is required, every effort will be made to contact me and/or my emergency contact. However, if either my emergency contact or I cannot be reached, I further consent to and authorize the giving of all treatments, medications, and performance of any other technical procedures, which are ordered by a physician for their care should they require medical attention as a result of their participation in this event sponsored by TFH. I also agree to accept all financial responsibilities incurred as a result any such treatment, medications or procedures and agree that, under no circumstances, will TFH, its officers, directors, employees and/or volunteers be financially responsible for the same.

• I understand that the transportation of my child to the event may involve an element of risk and danger of accidents. I understand that my child will be a passenger in the transportation vehicle. I am fully aware of risks and hazards associated with this. Knowing those risks, I hereby waive, release, and discharge TFH, its officers, directors, employees and/or volunteers from any and all liability, claims for damages for personal injury, death, or property damage, or any rights of recovery which I may have, or which may hereafter occur, as a result of their transportation to this event sponsored by TFH. Knowing those risks, I hereby agree to voluntarily assume responsibility for those risks, known or unknown, and whether caused by negligence or accident. I further agree to indemnify and hold harmless TFH from any loss, liability, damage or costs, including court costs and attorney’s fees, that may incur as a result of their transportation. Finally, I further agree that this waiver, release and assumption of risk is to be binding on my heirs and assigns.

• By signing this form (either in hardcopy or electronically), I am making an informed and knowing waiver as required by law. I acknowledge that I have carefully read this agreement, waiver, and release and fully understand its contents. I am aware that this release of liability is an agreement between myself and The Father’s House
